ISS Online Portal

All IS applications are now created online using the FAHCSIA Portal. All CCB approved services can access this portal using this link -  FaHCSIA Online Funding Management System ISS Case Portal

Essentially an application requires:

  • A completed and signed SSP (Service Support Plan) for the care environment uploaded to the on line IS application
  • A completed on line IS application submitted for endorsement when all service information has been completed and relevant documents uploaded.
  • A copy of each child's health care card OR current diagnosis letter (either/or) uploaded to the on line IS application
  • A copy of the Permission to share information signed by the parent/guardian of each child uploaded to the application.
  • If this is a reapplication then an evaluated SSP from the previous approval period will be required and uploaded to the IS case.

An Inclusion Support Facilitator can come out and assist with some of this process if you would like. Just contact our office for a date and time that suits.

If you are happy to complete the form yourself and submit it for endorsement please remember to send our office an email so we know to go in and check it ready for endorsement. Once we endorse the application we then submit it to the National Inclusion Subsidy Support Provider NISSP.  We are unable to endorse and submit an incomplete application so please check all relevant information before submitting it to us for endorsement.

If you have any queries regarding claims then these need to be addressed through your CCMS Portal and the National ISS Provider as the Inclusion Support Agency are unable to assist services with the claim side of the process. Our support is around assisting services with their application for IS support and ongoing support around quality inclusive programs.  All queries around enrolling children into your system need to be addressed through your service CCMS Portal.

The Telephone Hotline for the National Provider is 1800 824 955.
